TO MAKE A REFERRAL
 

First Step

To make a referral we request that you contact our office at the above number or email. You will be advised to send all referring reports and any clinical evaluations to the email address listed above. These documents can include but are not limited to: Psychological Evaluation, Psychiatric Assessment, Admission/Discharge assessments for hospital stays, and any other records that will give us information on current behaviors and needs. Once the documents are received, our staff will review to determine if our program will be able to meet the needs of the potential participant. We will process your referral as quickly as possible and you should hear back from us within 24 hours of receipt of documents.

Second Step
 

Once approved for admission, we will need a completed application form as well as specific documents and records in order to schedule the admission. Please click referral link below to complete our application form.
